Types of Physical Exam

The variation is due to the many different types of physicals. Some of which include:

  1. Annual/self/personal physical
  1. Pre-employment physical
  1. Program specific physical
  1. Sports Physical

 

Annual/self/personal physical is the most "basic".

  • Physical Exam intake form 
  • Vitals 
  • Vision Test 
  • Hearing Test 
  • Physical Exam 
  • +/- blood work/ annual health screening labs 

 

Pre-employment physicals

  • Same as personal physical
  • +/- TB test - skin tests require f/u in 48-72 hr for interpretation
  • +/- urine drug testing 
  • +/- employer specific form completion 
  • +/- specific vaccinations 

 

Program specific physicals

  • Same as personal physical
  • +/- Vivarium forms 
  • +/- OSHA Respirator form/eval 
  • +/- Nursing program forms 
  • +/- specific vaccinations 

 

Sports physical

  • Same as personal physical
  • NCAA Sickle Cell Screening (clinician order/lab)
  • Sports physical form (clinician)
  • +/- EKG (MA/clinician)
